REQUIRED DOCUMENTS

The two documents are required  when you come in for your visa medical examination.

  1. Valid Passport
    If not available, the following identification documents may be presented:

    • Certificate of Identity
    • Refugee Travel Document
  2. Letter/Medical instruction for Medical (if available)
  1. Minors (18 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child’s medical history.

Whenever applicable, please bring the following:

  • Old chest x-ray films not less than 3 months prior to the medical examination
  • Medical certificate regarding a previously-treated or currently managed clinically significant illness. The medical certificate should contain the following information:
    • Medical Treatment
      • name of medication (brand and/or generic name)
      • dosage
      • duration and dates of treatment
    • surgical procedure done
    • complete final diagnosis
    • outcome of treatment or treatment plans and prognosis, if available
    • Physicians’ affixed signature above printed name
    • Physicians’ clinic address, contact numbers and email address.

TB screening and treatment are included in the Full Medical Examination, while all non-TB tests will be charged accordingly.

Chest x-ray only – K 600.00
Specialty consultations/reports available within the clinic are charged accordingly.  Any other additional tests required by the specialists shall be individually charged.

WORKING FLOWCHART

REMINDERS:

  1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There is a café within PIH for food and drinking water.
  2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation. Otherwise, they will be required to come back one week after the last day of their menstruation.
  3. Minors (18 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child’s medical history.

Note:

  • Healthcases will be graded and uploaded online via the eMedical website according to the physical examination, x-ray and laboratory reports, which will be conducted and processed in accordance with the guidelines.
  • The medical report and chest x-ray are submitted by PIH electronically thru the eMedical website, to be directly submitted to the Immigration department.
  • Paperform medical reports and chest xray shall be released to the applicant, to be directly submitted to the Immigration Department.
